Notice of Adverse Action
Automate America, Inc. ("Automate America") has made a decision that adversely affects your ability to provide services as an independent contractor or professional on the Automate America platform.
This decision was based, in whole or in part, on information contained in a consumer report obtained from a consumer reporting agency.
Consumer Reporting Agency Information
The consumer report that was used in making this decision was provided by a consumer reporting agency. Their contact information is included with this notice.
IMPORTANT: The consumer reporting agency named in your notice did NOT make the decision to take adverse action against you and is unable to provide you with the specific reasons for this decision.
Right to a Free Copy of Your Consumer Report
You have the right to obtain a FREE copy of your consumer report from the consumer reporting agency.
- Request within 60 days of receiving this notice to receive a free copy
- You may also obtain a free copy once per year from www.annualcreditreport.com
Right to Dispute Inaccurate Information
If you believe any information in your consumer report is inaccurate or incomplete:
- You have the right to dispute that information directly with the consumer reporting agency
- The consumer reporting agency must investigate your dispute within 30 days
- If the information is found to be inaccurate, it must be corrected or deleted
- You have the right to add a statement to your file explaining any dispute
Right to Know What Is in Your File
You have the right to:
- Know all information in your consumer file
- Know who has requested your consumer report within the past year (2 years for employment purposes)
Right to Seek Damages
If a consumer reporting agency, user of consumer reports, or provider of information violates the FCRA, you may sue in state or federal court for:
- Actual damages
- Statutory damages (in some cases)
- Punitive damages (in some cases)
- Attorney's fees and costs
How to Dispute Information
Step 1: Contact the Consumer Reporting Agency
Contact the agency identified in your notice directly. You may submit a dispute online, by mail, or by phone.
Step 2: Provide Supporting Documentation
When disputing information, include:
- Your full name and address
- Identification of the specific information you are disputing
- An explanation of why the information is inaccurate
- Supporting documentation (if available)
Step 3: Await Investigation Results
The consumer reporting agency must:
- Investigate your dispute within 30 days
- Notify you of the results
- Provide a free copy of your updated report if changes are made
Reconsideration
If the consumer reporting agency corrects information in your consumer report as a result of your dispute, you may request that Automate America reconsider its decision.
