<p>In connection with your application for engagement as an independent contractor, or during the course of your engagement, <strong>Automate America, Inc.</strong> ("the Company") may obtain a "consumer report" and/or "investigative consumer report" about you from a consumer reporting agency.</p><p>This disclosure is being provided to you in compliance with the <strong>Fair Credit Reporting Act (FCRA)</strong>, 15 U.S.C. § 1681 et seq.</p>
What is a Consumer Report?
<p>A <strong>consumer report</strong> is a report that contains information about your:</p><ul><li>Credit history</li><li>Criminal history</li><li>Motor vehicle records</li><li>Employment history verification</li><li>Education verification</li><li>Professional license verification</li><li>Character, general reputation, and personal characteristics</li></ul><p>An <strong>investigative consumer report</strong> may also include information obtained through personal interviews with individuals who know you (neighbors, associates, etc.).</p>
Information That May Be Obtained
<p>The consumer report may include, but is not limited to:</p><ul><li>Criminal records (felony and misdemeanor convictions)</li><li>Sex offender registry checks</li><li>Driving records (MVR)</li><li>Social Security number verification</li><li>Address history</li><li>Employment history verification</li><li>Education verification</li><li>Professional reference checks</li><li>Drug testing results (if applicable)</li></ul>
How This Information Will Be Used
<p>The information obtained may be used to:</p><ul><li>Evaluate your suitability for engagement</li><li>Make decisions regarding your continued engagement</li><li>Comply with client requirements for background screening</li><li>Ensure workplace safety and security</li></ul>
Your Rights Under the FCRA
<p>You have the right to:</p><p>1. <strong>Request disclosure</strong> of the nature and scope of any investigative consumer report</p><p>2. <strong>Receive a copy</strong> of any consumer report used in making an adverse decision</p><p>3. <strong>Dispute</strong> the accuracy or completeness of any information in the report</p><p>4. <strong>Receive a summary of your rights</strong> under the FCRA</p><p>If information from the consumer report is used to make an adverse decision about your engagement, you will receive:</p><ul><li>Notice of the adverse action</li><li>Name and contact information of the consumer reporting agency</li><li>A copy of the consumer report</li><li>A summary of your FCRA rights</li></ul>
Consumer Reporting Agency
