  • W-9 (or W-8BEN) – Tax form with your legal name and taxpayer ID
  • ACH Authorization – Bank details for direct deposit
  • Voided Check or Bank Letter – Verification of bank account
  • Proof of Address – Utility bill, bank statement, or government letter (within 90 days)
  • Certificate of Insurance – Minimum $1M general liability coverage
